At a guess, I'd say the introduction of the database abstraction layer in
version 1.2.0 replaced the code that did the case-insensitive conversion.
I'm surprised we got this far without someone noticing.
The attached patch should fix the problem. It converts the domain name
parsed out of the header to lowercase before handing it to any database
query functions.
