You pipe ham or spam to it (in full header-and-body form) and it attempts to either update your local database or generate a statistics file entry to mark the message as spam/ham accordingly, so that the statistics about it are accurate.

It's a form of user input to the reputation system, rather than relying on something automatic like SpamAssassin.
